In this role, you will assess psychosocial needs, develop care plans, provide crisis intervention, connect patients and families with essential resources, and help overcome barriers to health and well-being. The Social Worker II also responds to urgent mental health needs, including positive suicide screenings, and collaborates closely with interdisciplinary teams to ensure coordinated, patient- and family-centered care.
