Coverage Area: Tallahassee, FL / Leon, Franklin, or Taylor Counties
(Territories designed to minimize drive time with clustered scheduling)
Compensation & Earning Potential
- Salary+ model: $88,000 – $95,000+ for 24 productivity points
- Pay-per-visit at $70+ per visit + Medicare differential pay for all visits over 24 points
- Quarterly bonus opportunities

Your Role as a Home Health Physical Therapist
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
