Iowa Home Care is looking for a Registered Nurse Case Manager to join our team and coordinate comprehensive home health services throughout Polk County. As an RN Case Manager, you’ll serve as a key point of contact for patients, families, physicians, caregivers, and members of the clinical team.
You’ll assess patient needs, establish individualized plans of care, monitor progress, and make sure services remain coordinated as a patient's needs change. Key Responsibilities Include
Complete comprehensive start-of-care, resumption-of-care, recertification, and ongoing patient assessments as required Evaluate physical, emotional, functional, and environmental factors affecting the patient's ability to remain safely at home Develop individualized plans of care based on patient needs, physician orders, and clinical findings Provide skilled nursing services, including medications, treatments, disease management, and other interventions within the scope of practice Monitor patient progress and identify changes that may require adjustments to the plan of care Communicate patient status and significant changes with physicians and other appropriate healthcare providers Obtain and document physician orders when changes in services or treatment are needed Coordinate nursing, therapy, aide, and other services involved in the patient's care Collaborate with physical therapists, occupational therapists, speech therapists, social workers, home health aides, and other members of the care team Educate patients and caregivers regarding medications, disease processes, treatments, safety, and strategies for managing care at home Involve patients and families in care planning and encourage active participation in achieving health goals Provide direction, instruction, and supervision to home health aides as required Complete required supervisory visits and evaluate the quality of services being provided Maintain thorough, accurate, and timely clinical documentation Ensure communication and care coordination remain consistent throughout the episode of care Advocate for patients and help identify resources that support their health, safety, and independence The Kind of Nurse We're Looking For We’re looking for an RN who is: Clinically Confident You can assess a patient's needs, recognize changes in condition, and make sound clinical decisions while knowing when additional support or physician involvement is needed.

Qualifications
Qualifications: Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting. Current RN licensure and CPR certification. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Requirements
