Job Details: Job Location: Wellness Clinic on Burnside - Portland, OR, Position Type: On-Call, Salary Range: $75.00 - $85.00Hourly, Job Shift: Day, Title: Family Nurse Practitioner - PRN (As Needed) Location: Indian Health Clinic - 703 NE Hancock St., Portland, OR 97212 & Wellness Center - 12360 E. Burnside St., Portland, OR 97233 Schedule: This is an in-person PRN / as-needed position, Monday through Friday.
Hours are flexible and can vary between 0 - 40 per week depending on availability and clinic needs.

Position Overview: The PRN Family Nurse Practitioner (FNP) serves as a provider of primary health care services, coordinating and referring clients for health care and health education while delivering comprehensive, patient-centered care.
This position provides clinical coverage on an as-needed basis and supports continuity of care in accordance with clinic needs, professional standards, and applicable licensing requirements.
This position functions in a PRN (as-needed) capacity, with scheduling and hours determined by clinic needs and patient care demands. What you will do: Provide comprehensive primary health care services on an as-needed basis to support clinic operations, including provider absences and fluctuations in patient demand.
Coordinate patient care with other providers and agencies and advocate for clients within health care systems. Provide services in prenatal integrated health care of high-risk chemically dependent pregnant patients, in consultation and/or collaboration with obstetrical health care providers.
Perform initial and ongoing health risk assessments and counseling for high-risk lifestyle behaviors and/or familial diseases, including ongoing drug and alcohol assessment and HIV screening. Encourage family participation in the health care of patients as deemed appropriate by the patient and other clinic providers.
Develop and document plans of care and outcomes evaluations. Coordinate with contractual pharmacy, primary care providers, hospitals, and laboratories for required diagnostic procedures and delivery/births.
Provide health education to individual patients as needed, including birth control, safer sex, high-risk behaviors, and associated health consequences. Coordinate the care patients receive from other providers and agencies and make referrals as appropriate.
Engage in ongoing education and specialized training in order to maintain a high quality of care to the Clinic's patient population. Attend meetings and participate in clinic activities as requested, as needed.
Document patient care accurately and appropriately in accordance with organizational policies and professional standards.
Qualifications
We would like to hear from people that have: Graduation from an accredited Nurse Practitioner program with certification as a Family Nurse Practitioner (FNP) or related primary care specialty. Nurse Practitioner, unencumbered licensure with State of Oregon as Certified Nurse Practitioner and prescriptive authority.
Current DEA registration or ability to obtain upon hire. Basic Life Support within introductory period. Evidence of experience sufficient for successful performance of duties noted above. Must pass a pre-employment or for cause drug tests. Must pass criminal background and DMV checks.
Preferred Qualifications
Experience working within the Native American/Alaska Native community Prior experience in primary care, family practice, community health, or federally qualified health center setting.
