Are you a Licensed Social Worker (LSW or LISW) looking for meaningful work where you can support patients and families during some of life’s most important moments?
Ohio’s Hospice is seeking a compassionate and dedicated Medical Social Worker to join our inpatient hospice team in Newark, Ohio. This position provides psychosocial support, counseling, advocacy, resource coordination, caregiver support, and end-of-life care to hospice patients and their families in a primarily hospital-based setting.
You’ll work closely with nurses, physicians, chaplains, hospice aides, hospital staff, and other members of the interdisciplinary care team to provide compassionate, patient-centered care focused on comfort, dignity, choice, and quality of life.
Schedule & Work Environment
- Monday–Friday | 8:30 a. m.–5:00 p. m.
- Shared weekend and holiday rotation
- On-call rotation following completion of orientation
- Primarily hospital-based inpatient hospice work
- Based in Newark, Ohio
- Driving is required as part of the position; however, no routine community-based travel is required for this inpatient role
- Close collaboration with an interdisciplinary hospice and hospital care team

Position Summary
As a Medical Social Worker on the inpatient hospice team, you will provide comprehensive psychosocial services to hospice patients and their families. You will assess patient and family needs, provide counseling and emotional support, coordinate resources, advocate for patient preferences, and assist families as they navigate end-of-life decisions and transitions.
You will serve as an important member of the interdisciplinary team, helping address the emotional, social, financial, and practical concerns that may arise during an inpatient hospice stay.
Essential Responsibilities
- Conduct psychosocial, emotional, financial, and practical assessments of patients and families.
- Develop and participate in individualized plans of care based on patient goals, preferences, and needs.
- Provide counseling, education, emotional support, and crisis intervention.
- Support patients and families as they navigate end-of-life decisions and changes in condition.
- Provide caregiver support related to stress, anticipatory grief, loss, and bereavement.
- Assist patients and families with advance directives and end-of-life planning.
- Connect patients and families with financial assistance, community resources, agencies, and support programs.
- Advocate for patient rights, preferences, dignity, comfort, and quality of life.
- Participate as an active member of the interdisciplinary hospice team.
- Collaborate with physicians, nurses, chaplains, hospice aides, volunteers, hospital staff, and other healthcare professionals.
- Participate in patient and family care conferences and interdisciplinary discussions.
- Facilitate communication between patients, families, and members of the care team.
- Assist with discharge planning and coordination of appropriate resources and services when applicable.
- Complete accurate and timely documentation in the electronic medical record.
- Maintain professional relationships and effective communication with patients, families, hospital partners, and members of the interdisciplinary team.
- Participate in the shared weekend, holiday, and on-call rotation as scheduled.
What You Bring
- Current LSW or LISW licensure as required.
- Social work experience in healthcare, hospice, palliative care, hospital, or a related setting is valued.
- Strong communication, counseling, advocacy, and interpersonal skills.
- Ability to support patients and families experiencing grief, loss, crisis, and significant life transitions.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Strong organizational, documentation, and time-management skills.
- Ability to respond appropriately to changing patient and family needs.
- Valid driver’s license and ability to drive as required by the position.
