About the Role

Join Ohio s Hospice Inc as a Medical Social Worker – FT Days, On-Call in United States. What You Should Know About the Social Worker Role: * The Social Worker would be for coverage under Ohio's Hospice South regions * This is a full-time position. M-F, 8A-5P with after hours/weekend on call * We provide superior care and superior services to patients at their end of life journey.

Requirements

Licensed Social Worker with the state of Ohio LISW preferred or LSW.

Master's Degree in Social Work or related degree as required for LSW licensure.

One year social worker experience in a health care setting preferred.

previous discharge planning experience
