The RN Case Manager plans, organizes, and provides hospice care while building community resources to meet the needs of patients and families. They also provide clinical direction to the interdisciplinary team and supervise hospice aides and licensed practical nurses.
Candidates must be a graduate of an accredited nursing school and hold a current state license as a Registered Nurse. A minimum of two years of clinical, home health, or hospice experience is preferred, along with a valid driver's license and reliable transportation.
