This position offers a signing bonus. Work with Affinity Hospice in Florence, Alabama.
The Licensed Master Social Worker assesses the psychosocial needs of hospice patients and their families to develop and implement effective care plans. They also provide supportive counseling, facilitate community referrals, and participate in interdisciplinary team meetings to ensure comprehensive patient support.
Candidates must hold a Master's degree in Social Work and possess a professional licensure as a Master Social Worker in the state of employment. Additionally, clinical experience or formal training in death, dying, and bereavement is required, along with a valid driver's license.
