Key Responsibilities
The RN Case Manager plans, organizes, and provides hospice care while directing the interdisciplinary team and family members. They are responsible for developing patient care plans, ensuring compliance with third-party payers, and supervising hospice aides and licensed practical nurses.
Requirements
Candidates must hold a degree from an accredited nursing school and possess a current state Registered Nurse license and CPR certification. A minimum of two years of clinical, home health, or hospice nursing experience is preferred, along with a valid driver's license and reliable transportation.
