Position Details
Full-time, Monday–Friday
Report to the office at 8:00 AM to pick up the company vehicle for scheduled visits
Complete patient visits and case management responsibilities as scheduled
Work collaboratively with patients, families, caregivers, and the clinical team to ensure quality care and continuity of services
Qualifications
Current, valid Registered Nurse (RN) license
Experience in home health preferred
At least one to three years of case management experience preferred
Strong communication, organizational, and critical-thinking skills
Compassionate, dependable, and patient-focused
