Key Responsibilities

The Nurse Practitioner will conduct Medicare Wellness Visits, manage chronic and acute patient care, and participate in quality improvement initiatives. They are also responsible for maintaining accurate clinical documentation and collaborating with the medical team to ensure high-quality patient outcomes.

Requirements

Candidates must hold a Bachelor's degree and be a graduate of an accredited Nurse Practitioner program with a valid Colorado license. Proficiency in Spanish and at least one year of experience in ambulatory health care are preferred.
