Qualifications
- ptom management in accordance with hospice standards, physician orders, and individualized plans of care. Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB). Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care. Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes. Educate patients, families, and caregivers on sym

Key Responsibilities
The Hospice RN Case Manager provides skilled nursing care, symptom management, and individualized care plans for patients in their homes. They also collaborate with an interdisciplinary team and educate families on end-of-life care and disease progression.
Requirements
Candidates must hold an active RN license and possess strong assessment and care coordination skills. Experience in hospice nursing and proficiency with Homecare Homebase software are preferred.
