Key Responsibilities
The RN Case Manager performs patient assessments and collaborates with the care team to develop and implement individualized care plans. They also provide patient and caregiver education while maintaining accurate progress records and performing on-call duties.
Requirements
Candidates must have at least one year of nursing experience and a current RN license for the assigned state. A current CPR certification and a valid driver's license with reliable transportation are also required.
