Key Responsibilities

The Nurse Practitioner will provide advanced palliative care to patients in their homes, performing assessments and developing individualized care plans. They will collaborate with the Palliative Medical Director to manage complex medical needs and guide patients through treatment stages.

Requirements

Candidates must hold a Master of Science in Nursing and have at least two years of Nurse Practitioner experience. A current RN license, Nurse Practitioner certification, and a valid driver's license are required.
