Key Responsibilities
Conduct in-home annual wellness visits and chronic condition assessments for diverse patient populations. Perform focused physical exams, medication reviews, and screenings while documenting findings in the EMR to support value-based care.
Requirements
Requires an active, unencumbered Nurse Practitioner or Physician Assistant license in the state of Michigan. Candidates should have at least 3 years of patient care experience and be comfortable with independent field-based work.
