Key Responsibilities
The Nurse Practitioner will provide high-quality, comprehensive follow-up care to patients post-hospital discharge via phone and video visits. Responsibilities include medication reconciliation, symptom screening, and coordinating care across various healthcare settings.
Requirements
Candidates must be a Certified Nurse Practitioner in Adult, Family, or Geriatrics with an active Connecticut license and at least 5 years of post-graduate clinical experience. Proficiency in virtual care delivery and experience engaging Medicare and Medicaid populations are required.
