Key Responsibilities
Perform virtual preventative care screenings, medication reconciliation, and health coaching for insurance members. Identify and address gaps in care while cross-training to support urgent and primary care service lines.
Requirements
Requires a PA, DNP, FNP, or ENP degree from an accredited program with current board certification. Candidates must have at least 3 years of experience in outpatient primary care and hold specific state licensures.
