Key Responsibilities

The Transitional Care Nurse Navigator facilitates patient progress through acute care episodes and coordinates transition plans from inpatient to post-acute settings. They serve as a patient advocate and foster communication between the multidisciplinary team, physicians, and families.

Requirements

Candidates must hold a Bachelor of Science in Nursing (BSN) and possess at least 3 years of acute care nursing experience. A Pennsylvania Registered Nurse license is required, and Certified Case Manager status is preferred.
