Occupational Therapist – Help Patients Live Safely and Independently at Home!

Are you an Occupational Therapist who enjoys helping people regain independence, improve daily function, and remain safely in the place they call home? Home Healthcare, Hospice & Community Services is looking for an Occupational Therapist to provide skilled, individualized care to patients in their homes.

In this role, you’ll evaluate patient needs, develop and implement individualized plans of care, provide skilled occupational therapy interventions, and collaborate closely with patients, families, caregivers, and an interdisciplinary healthcare team. Your work will help patients improve function, adapt to changing abilities, and achieve meaningful goals in their everyday lives.

Key Responsibilities

Conduct comprehensive occupational therapy evaluations and ongoing assessments of patients’ physical, functional, psychosocial, cognitive, and environmental needs.

Develop individualized, measurable plans of care based on assessment findings, physician orders, patient goals, and evidence-based practice.

Provide skilled occupational therapy interventions to improve or maintain functional abilities, promote independence, and achieve established outcomes.

Address activities of daily living (ADLs), instrumental activities of daily living (IADLs), mobility, transfers, safety, adaptive techniques, energy conservation, positioning, cognition, and other areas within the occupational therapy scope of practice.

Evaluate patient progress and modify plans of care based on changing needs, progress, and barriers to achieving goals.

Assess home environments for safety concerns and recommend appropriate modifications, adaptive equipment, and community resources.

Educate patients, families, and caregivers on therapeutic activities, adaptive techniques, equipment use, safety, symptom management, and strategies that promote independence.

Communicate significant changes in patient condition, progress, concerns, and recommendations to physicians and members of the interdisciplinary team.

Coordinate with the care team to promote safe, effective, and uninterrupted services.

Maintain timely, accurate, and complete clinical documentation in accordance with agency, payer, regulatory, and electronic health record requirements.

Protect patient confidentiality and electronic protected health information and follow applicable infection prevention, safety, compliance, and regulatory requirements.

Maintain established productivity expectations, including an average of approximately 4–6 patient visits per day, as applicable to caseload and operational needs.

Work Environment & Physical Requirements

Work is performed primarily in patients’ homes and requires travel between multiple locations throughout the workday.

Must be comfortable working in varied home environments and weather conditions and following appropriate infection prevention, safety, and personal protective equipment practices.

Requires the ability to sit, stand, walk, bend, stoop, crouch, kneel, reach, climb stairs, and maintain balance while performing job duties.

Requires the ability to assist with and demonstrate safe transfers and mobility techniques and to push or pull therapeutic and assistive equipment.

Must be able to lift and carry equipment and supplies weighing up to approximately 50 pounds, with heavier or more complex transfers requiring appropriate assistance or additional caregivers.

Requirements

Required

  • Graduate of an accredited occupational therapy program recognized by the appropriate state and national regulatory bodies.
  • Current, unrestricted New Hampshire Occupational Therapist license.
  • Knowledge of occupational therapy principles, assessment techniques, treatment interventions, adaptive equipment, and home safety.
  • Ability to work independently and effectively manage an assigned caseload.

Strong communication skills and the ability to work effectively with patients, families, caregivers, physicians, and interdisciplinary team members.

Ability and willingness to use electronic health records, portable electronic devices, and other technology required for documentation and communication.

Ability to appropriately manage electronic protected health information in accordance with privacy requirements and agency policies.

Valid driver’s license and reliable access to an automobile during working hours that meets State of New Hampshire registration and auto liability insurance requirements.

Preferred

Previous professional occupational therapy experience preferred; new graduates may be considered based on demonstrated competency and applicable supervision requirements.
