Key Responsibilities

Oversee the daily operational workflow of the Meds to Beds program and transitions of care staff to ensure smooth hospital discharges. Develop operational policies to address medication access barriers and implement wraparound services to prevent readmissions.

Requirements

Requires a Doctor of Pharmacy degree and at least one year of experience in a hospital-based ambulatory or transitions-of-care setting. Must have experience coordinating multidisciplinary teams and managing insurance prior authorization workflows.
