The OP Ambulatory Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations. She/He applies specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. Participates as a contributing member of ambulatory care teams such as rheumatology, dermatology, neurology, infectious disease, oncology, nephrology, or pulmonology. Provides clinical, pharmacy, and medication expertise in each area including pharmacy operations, prior authorization support and medication access, and medication therapy outcomes.

Education

Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred. Licensed pharmacist in the State of Vermont.

Experience

3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.

The pharmacist ensures optimal drug use outcomes for high-risk and complex patient populations through specialized pharmacotherapy knowledge. They collaborate with ambulatory care teams across various specialties to provide clinical expertise and medication therapy management.

Requirements

Requires a Doctor of Pharmacy degree and a valid Vermont pharmacist license, with a preference for an ASHP accredited residency. Preferred experience includes 3 years of advanced practice in ambulatory care or a clinical outpatient setting.
