Pyramid Healthcare is seeking a Qualified Professional (QP) to join our Assertive Community Treatment Team (ACTT) in Mars Hill, NC.

ACTT is a “hospital without walls,” providing intensive, community-based behavioral health services to individuals living with severe and persistent mental illness (SPMI). Rather than requiring individuals to come to a traditional treatment setting, our multidisciplinary team brings services directly into the community.

As an ACTT QP, you'll help coordinate care and ensure individuals receive the services, resources, and support they need to build greater stability and independence. You'll work closely with clients, families, psychiatric providers, referral sources, and other members of the ACTT team.

This position is particularly well suited for someone with experience in community mental health, case management, vocational rehabilitation, housing services, or working with individuals with serious mental illness.

What You'll Do

Care Coordination & Treatment Planning

  • Develop and maintain person-centered treatment plans based on each individual's needs, goals, and level of care.
  • Coordinate services with clients, families, providers, and community partners.
  • Facilitate regularly scheduled treatment team meetings and participate as an active member of the multidisciplinary ACTT team.
  • Review treatment plans with the team and document progress toward identified goals.
  • Develop discharge and transition plans that promote continuity of care and long-term stability.

Utilization Review & Referral Coordination

  • Collect and review information related to client services and treatment needs.
  • Coordinate initial and ongoing authorization requirements with applicable LME/MCOs, third-party payers, and referral sources.
  • Prepare and participate in concurrent reviews as required.
  • Communicate client progress, treatment needs, and discharge planning information to referral sources and other authorized partners.

Community-Based Support

  • Provide services in homes, community settings, and other locations throughout the service area.
  • Help individuals connect with housing, vocational, social, and community resources.
  • Support clients in developing skills that promote independence and community integration.
  • Provide transportation to appointments, services, and community resources when needed.
  • Build professional, supportive relationships with individuals while maintaining appropriate boundaries.

Documentation & Quality Improvement

  • Complete timely and accurate documentation in the electronic health record.
  • Maintain documentation of client contacts, treatment services, and coordination with outside providers.
  • Complete required treatment plan reviews, discharge documentation, and clinical records.
  • Participate in Continuous Quality Improvement (CQI) activities and peer record review.
  • Complete required NCTOPPS data entry and other program documentation.

Crisis & Emergency Services

  • Participate in the ACTT emergency/crisis response rotation.
  • Assist with crisis planning, after-hours assessments, and hospital diversion.
  • Participate in commitment-related services and emergency interventions as required by the program.
  • Respond appropriately to changing client needs while maintaining safety and professional standards.

What We're Looking For

Required

Bachelor's degree in Social Work, Counseling, Psychology, Human Services, or a related field.

Must meet Qualified Professional (QP) requirements under 10A NCAC 27G.0104. North Carolina defines QP status based on applicable education, credentials, and experience requirements.

  • Valid North Carolina driver's license and acceptable driving record.
  • Ability to provide reliable transportation for community-based services.
  • Ability to work effectively with individuals experiencing serious mental illness.

Preferred

  • 1–3 years of experience working with individuals with severe and persistent mental illness (SPMI).
  • Master's degree in Human Services or a related discipline.
  • Experience with ACTT or other community-based behavioral health programs.
  • Experience in vocational rehabilitation, supported employment, housing services, or case management.
  • Knowledge of community resources and behavioral health service systems.
