The Nurse Practitioner or Physician Assistant will provide personalized primary care to seniors and individuals with complex medical needs in their residences. Responsibilities include conducting clinical assessments, developing care plans, and collaborating with interdisciplinary teams to manage chronic and acute health conditions.
Candidates must hold an active state license as a Nurse Practitioner or Physician Assistant, along with current CPR and DEA certification. A Master's degree in Nursing is required for NPs, and at least five years of nursing experience is expected, with a preference for two years in an NP or PA role.
