The Nurse Practitioner will conduct clinical assessments and develop personalized care plans for seniors and individuals with complex medical needs in their residences. They will collaborate with interdisciplinary teams to manage chronic conditions and ensure seamless care transitions.
Candidates must hold an active state Nurse Practitioner or Physician Assistant license, a Master's degree in Nursing for NPs, and current CPR certification. A minimum of five years of healthcare experience is required, with at least two years of experience as an NP or PA preferred.
