The Physical Therapist will evaluate and develop individualized treatment plans to restore function and improve mobility for patients in their homes. They will also collaborate with an interdisciplinary team to ensure comprehensive care and provide education to patients and their families.
Candidates must hold a current state licensure as a Physical Therapist and be a graduate of an accredited Physical Therapy program. While home health experience is preferred, it is not strictly required for this position.
