About Us
Home Health Works has cared for Pinellas County families since 1992. We are a licensed skilled and non-skilled home health agency serving private pay clients, VA Community Care Network veterans, and Area Agency on Aging ADI and CCE program participants. We are expanding our skilled nursing service line and need a clinical leader to build it. We want a clinically experienced DON who is equally energized by clinical leadership and by building referral relationships at hospitals and VA facilities.
What You'll Do
- Clinical Leadership
- Oversee all skilled and non-skilled clinical operations
- Supervise and mentor field RNs and LPNs
- Review and approve plans of care and OASIS documentation
- Ensure AHCA compliance and maintain year-round survey readiness
- Conduct supervisory visits and caregiver competency evaluations
- Manage VA CCN skilled case oversight and authorization compliance
- Serve as the AHCA-designated DON of record
- Business Development
Build and manage referral relationships at Morton Plant Hospital, HCA Florida Largo, and Bay Pines VA Medical Center
- Visit hospital case managers, discharge planners, and wound care clinics in person
- Develop orthopedic and primary care physician referral pipelines
- Represent the agency at community events, senior fairs, and VA provider networking opportunities
- Track referral metrics and report skilled pipeline growth to leadership monthly
Required Qualifications
- Current Florida RN license in good standing
- Minimum 3 years as a DON or ADON in a home health setting
- Proven track record growing a skilled nursing service line
- Working knowledge of Florida AHCA home health regulations (Rule 59A-8)
- Demonstrated success building referral relationships with hospitals and physician practices
- Ability to clear Florida AHCA Level 2 background screening before start
- Preferred
- Experience with VA CCN documentation, HCPCS coding, and Availity billing

- Background Screening
This position requires Florida AHCA Level 2 background screening (FDLE and FBI fingerprinting) before hire. All offers are contingent on satisfactory clearance. Screening costs are covered by Home Health Works.
Location
Office-based in Clearwater, FL, with regular local travel throughout Pinellas and Pasco counties for partner visits and community events.
