Key Responsibilities

  • Coordinate referrals and ongoing access to primary and other health care services for patients.
  • Provide consultation to and seek consultation from other health care clinicians to enhance quality and cost effective services for patients.
  • Maintain patient's electronic medical record and accurately document each patient encounter. SUPPLEMENTAL FUNCTIONS:

  • EDUCATION: Master’s degree in Nursing
