On-Call Nurse Practitioner Position Summary
The On-Call Nurse Practitioner (NP) provides after-hours clinical coverage for patients across the region, including nights, weekends, and holidays, as assigned.
This position is performed remotely through telephonic communication, electronic medical records usage, and clinical consultation with onsite healthcare staff. The On-Call NP is responsible for assessing, triaging, and managing urgent and emergent patient care needs that arise outside of regular business hours.
The role requiressound clinical judgment, rapid decision-making, effective communication, and collaboration with nursing staff, physicians, and operational leadership to ensure safe, timely, and appropriate patient care.
Duties and Responsibilitie
On-Call Nurse Practitioner Essential Functions
- Triage urgent and emergent patient care concerns via telephone in collaboration with onsite nursing staff.
- Confirm assessment findings and determine the appropriate level of care, including continued monitoring, treatment,
provider follow-up, or transfer to a higher level of care.
- Assess acute patient care needs and develop treatment plans consistent with the after-hours scope of practice.
- Provide verbal, telephone, and/or EMR (Electronic Medical Record) orders for licensed onsite staff in accordance with facility policies and regulatory requirements.
- Review and authenticate all verbal and telephone orders in a timely manner.
- Manage urgent medication and treatment needs.
- Reviewing patients transferring from one location to another (e. g. intake, return from hospital/ER, intakes). Reviewing reports or records that are available. Ordering treatments and medications as indicated.
- Review and approve urgent (those not appropriate to wait until normal business hours) non-formulary medication ordersplaced by On-Call NP or as requested by nursing.
- Complete, review, and correct EMR documentation and orders as instructed.
- Communicate clear treatment recommendations and follow-up instructions to onsite clinical staff.
- Identify patient care concerns requiring follow-up and ensure appropriate communication to the care team responsible.
- Escalate cases involving significant clinical uncertainty, potential mortality, sentinel events, or circumstances exceeding NP scope of practice to the On-Call Physician or Statewide Medical Director.
- Attend required meetings, training, and educational activities
- Review and respond to email correspondence in a timely manner.
- Perform other duties assigned within the scope of after-hours on-call practice.
- Follow all organizational policies regarding controlled substances, high-risk medications, restraints, emergency transfers, and mandatory reporting.
On-Call Nurse Practitioner Minimum Requirements
- Independently licensed with at least 5 years of experience.
- Graduate of an accredited college / university with current certification / licensure in the state of employment.
- Maintains required annual licensure and credentialing requirements for a Nurse Practitioner.
- Maintains an active CPR certification.
- Two years of work as a Nurse Practitioner, preferably in a correctional facility.
- Must be fluent in English language, including reading, writing, speaking, and understanding.
- Must be a supportive team member, who contributes to and demonstrates teamwork and team concept.
- Able to make independent decisions when circumstances warrant such action.
- Able to deal tactfully with personnel, family members, visitors, government agencies/personnel.
- Has strong and stable telephone and internet access during shifts.
- Possess leadership and supervisory ability and be able to work harmoniously with and supervise other personnel.
- Must not pose a direct threat to the health or safety of other individuals in the workplace
- Ability to appropriately prioritize urgent and non-urgent clinical issues.
- Ability to collaborate effectively with nursing staff, physicians, and interdisciplinary teams
