Provide comprehensive virtual primary care visits, including chronic disease management, preventive screenings, and acute care interventions. Collaborate with a multidisciplinary team to coordinate diagnostics, specialist referrals, and individualized care plans.
Requires an active Nurse Practitioner or Physician Assistant license with board certification in Family Medicine, Adult-Gerontology, or Internal Medicine. Candidates must have at least 2 years of experience in primary care, urgent care, or internal medicine.
