The Primary Care Physician is responsible for delivering high-quality, patient-centered care to a panel of up to 450 older adult patients. They will lead care teams, act as a health coach, and ensure compliance with healthcare guidelines to drive positive patient outcomes.
Candidates must hold an MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. A current, active State Medical License and proficiency in Microsoft Office Suite are mandatory requirements for this role.
