Role Overview: This role involves providing physical therapy services to patients in their homes to improve mobility, strength, balance, coordination, and overall physical function. The Home Health Physical Therapist plans and administers medically prescribed physical therapy treatments for patients suffering from injuries, or muscle, nerve, joint, and bone diseases, in an effort to restore function, relieve pain, and prevent disability by performing the following duties: providing education and information to patients and caregivers; participating in required discharge planning, and completing all required documentation according to policy; and directing patient participation in selected tasks to restore, reinforce, and enhance performance.
Requirements
- 1 year of experience required - Home Health experience preferred.
- Flu Vaccine Required - Medical/Religious Exemptions and Declinations Allowed.
- 1st time travelers accepted.
- MatrixCare or Epic experience preferred, not required.
- VA PT State License Required – Can be Pending.
- Certifications Required: CPR
Schedule: Shift: Monday – Friday 8:00 AM to 5:00 PM
Weekend: PT every 4th or 5th weekend
Key Responsibilities
The Physical Therapist will provide home-based care to improve patient mobility, strength, and overall physical function. Responsibilities include administering prescribed treatments, educating patients and caregivers, and completing all required clinical documentation.
