H2 Health is seeking a Physical Therapistto join our outpatient team in Dublin, VA.

At H2 Health, you'll work with a diverse outpatient population while providing individualized, evidence-based care. Our clinicians treat orthopedic injuries, post-surgical conditions, musculoskeletal disorders, balance and vestibular impairments, and mobility-related challenges.

Your Role
Perform comprehensive patient evaluations and develop individualized plans of care.
Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
Treat a variety of orthopedic, post-operative, musculoskeletal, balance, vestibular, and mobility-related conditions.
Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
- Educate patients on injury prevention, wellness, and home exercise programs.
- Maintain accurate and timely documentation of patient progress and treatment outcomes.
- Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
- Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
- Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
- Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
- Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.
- Current Virginia Physical Therapist license or eligibility for licensure.
- CPR certification or willingness to obtain as required.
- Ability and willingness to travel between assigned clinic locations.
