Provide 1-on-1 physical therapy treatment to patients in their homes. Utilize care maps and electronic medical record systems to document patient progress and treatment plans.
Must be a graduate of an APTA-approved two-year program and hold an active Virginia PTA license. Two years of home health experience and familiarity with HCHB EMR are preferred.
