Job Description
What You’ll Do
- Provide in-home primary care for chronic disease management, dementia care and preventive healthcare
- Perform cognitive screenings, annual wellness visits, follow-ups, and hospital-to-home transition supervision
What You Bring
- Willingness to travel to patient’s homes
- Active Virginia Nurse Practitioner
- DEA registration or eligibility
- Medicare PECOS enrolled
- Experience in primary care and geriatrics
