Job Description
First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant for our primary care practice in East Hartford, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting.

Experience and Training
- Proven experience as a Nurse Practitioner or Physician Assistant establishing care plans and managing patient outcomes
- A current license to practice as a NP or PA in the state of Connecticut.
- Appropriate Board Certification
- A commitment to serving diverse and vulnerable populations with compassion and respect.
- Excellent communication, interpersonal, and organizational skills.
- Connecticut Controlled Substance license and DEA license
- BLS Certification through the American Heart Association or American Red Cross.
- Pay is based on several factors including but not limited to work experience certifications, etc. Range for this position is $115,000 - $160,000
Standard Job Duties
- Evaluates, assesses patient illness/injuries.
- Chronic care management
- Performs physical examinations, and special examinations.
- Takes medical histories.
- Develops plans of care.
- Provides routine and illness care.
- Orders medications; orders diagnostic tests; and evaluates results.
- Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary.
- Maintains patient medical records.
- Provides patient counseling/education.
- Participates in medical student and health professional teaching.
- Performs related duties as required.

They are also responsible for ordering diagnostic tests, providing patient education, and maintaining accurate medical records.
Requirements
Candidates must hold a current Connecticut license as a Nurse Practitioner or Physician Assistant, along with appropriate Board Certification. Additionally, a DEA license, Connecticut Controlled Substance license, and BLS certification are required.
