*$95,000 starting annual salary for new grads, up to $150,000 annual salary depending on experience!

*This is a full-time position providing care to patients in an assigned service territory within the greater Phoenix metro area.

Key Responsibilities

• Develop and update individualized care plans
• Provide physical therapy services per physician’s plan of care
• Assess functional status, mobility, and home environment
• Educate patients and families on therapy techniques and equipment
• Maintain accurate documentation and communicate progress to physicians

Requirements

• Licensed Physical Therapist in the State of Arizona
• Degree from a CAPTE-accredited program; passed state-approved exam
• Current CPR certification
• Valid driver’s license, insurance, and reliable transportation
