Key Responsibilities

Conduct comprehensive in-home health evaluations for health plan members, including medication reviews and physical assessments. Build trusted relationships with patients to bridge gaps in care and provide personalized health education.

Requirements

Must hold an active, unrestricted Nurse Practitioner or Physician Assistant license in the coverage area. Candidates must be board certified and capable of traveling up to 100% of the time to perform duties in diverse home environments.
