Key Responsibilities

Provide advanced palliative care to patients in their homes through comprehensive assessments and individualized care plans. Collaborate with the Palliative Medical Director and interdisciplinary teams to manage complex medical needs and guide families through treatment.

Requirements

Requires a Master of Science in Nursing, a current RN license, and Nurse Practitioner certification with at least two years of experience. Must possess a DEA license and a valid driver's license, with ACHPN certification preferred.
