Key Responsibilities

The Nurse Practitioner will provide advanced palliative care to patients in their homes, collaborating with the medical director to manage complex medical needs. Responsibilities include performing patient assessments, developing individualized care plans, and prescribing medications.

Requirements

Candidates must hold a Master of Science in Nursing and have at least two years of experience as a Nurse Practitioner. A current RN license, Nurse Practitioner certification, and a valid driver's license are required.
