Key Responsibilities
The Nurse Practitioner will perform comprehensive health assessments, diagnose, and manage acute and chronic conditions for residents in skilled nursing facilities. They will collaborate with an interdisciplinary team to develop evidence-based care plans and improve patient outcomes.
Requirements
Candidates must hold a Master’s degree in Nursing and an active, unrestricted Advanced Practice Nurse license. At least 2 years of clinical experience in geriatrics, long-term care, or post-acute settings is preferred.
