Key Responsibilities
The Nurse Practitioner provides primary care, acute illness evaluation, and comprehensive assessments for PACE participants in both center and home settings. They collaborate with an interdisciplinary team to develop care plans, manage nursing home care, and participate in rotating night and weekend calls.
Requirements
Candidates must be a graduate of an accredited nursing school with a valid RN license, Nurse Practitioner certification, and prescriptive authority. A minimum of one year of experience in adult chronic disease management and a current DEA license are required.
