Job Description

We are seeking a knowledgeable, compassionate, and motivated Registered Dietitian to join our team! This position offers the opportunity to make a meaningful difference in the lives of older adults by providing individualized nutrition services and supporting quality dining experiences across multiple levels of care.

The ideal candidate is organized, detail-oriented, and passionate about resident-centered care. This role involves collaborating with nursing, dining services, healthcare providers, and leadership to promote nutritional well-being and maintain regulatory compliance.

Key Responsibilities

Complete nutritional assessments and reassessments, identifying residents at risk for nutritional deficiencies, weight changes, or dehydration.

  • Develop individualized nutrition recommendations, interventions, and follow-up plans.
  • Monitor resident weights, dietary intake, hydration, and nutritional concerns.
  • Collaborate with interdisciplinary teams to develop and update resident care plans.
  • Provide nutritional input for MDS assessments and care planning as applicable.
  • Review menus, therapeutic diets, supplements, and meal accommodations to ensure nutritional adequacy.
  • Support residents with specialized dietary needs, including those requiring modified textures, nutritional supplements, or enteral nutrition.
  • Provide nutrition education to residents, families, and staff.
  • Work collaboratively with dining services to promote food quality, resident satisfaction, and individualized dining experiences.
  • Maintain accurate and timely documentation in electronic medical records.
  • Assist with quality improvement initiatives, regulatory surveys, and compliance activities.
  • Provide nutrition recommendations and dining strategies tailored to individuals living with dementia.

Qualifications

  • Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
  • Active Pennsylvania Licensed Dietitian-Nutritionist (LDN) license required.
  • Degree in nutrition, dietetics, or a related qualifying field.
  • Previous experience in long-term care, skilled nursing, or senior living preferred.
  • Knowledge of therapeutic diets, nutritional assessments, and individualized care planning.
  • Familiarity with CMS and Pennsylvania healthcare regulations preferred.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Experience with electronic medical records preferred.
