Key Responsibilities

Conduct in-home health evaluations for health plan members, including medication reviews, physical assessments, and diagnostic screenings. Facilitate connections to appropriate care and maintain detailed clinical documentation using an iPad-based workflow.

Requirements

Must hold an active, unrestricted MD or DO license in the coverage area. Candidates must be comfortable performing clinical duties in diverse home environments.
