Key Responsibilities

The Physical Therapist will evaluate patient function, develop and implement individualized plans of care, and perform skilled therapy procedures in a home health setting. They are also responsible for supervising assistants, documenting patient progress, and coordinating care with physicians and family members.

Requirements

Candidates must hold a valid physical therapy license and have graduated from an accredited physical therapy program. Two years of experience is preferred, along with the ability to exercise independent judgment and maintain current CPR certification.
