Key Responsibilities

The Primary Care Physician is responsible for delivering high-quality, patient-centered care to a panel of up to 450 patients, focusing on health coaching and managing acute and chronic illnesses. They will lead care teams, ensure compliance with healthcare guidelines, and collaborate cross-functionally to drive positive patient outcomes.

Requirements

Candidates must hold an MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty, along with an active State Medical License. Proficiency in Microsoft Office Suite and experience with evidence-based medicine are required for this role.
