Job Description

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

Nurse Practitioner

The Nurse Practitioner provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families and other healthcare practitioners.

The Nurse Practitioner will conduct face-to-face visits, sign E-scripts, attend IDT meetings and assist members of a team in caring for patient/family needs. The Nurse Practitioner will travel to patients’ homes, nursing homes or long term care facilities to assist patients.

This position works typically Mon - Fri 8 am - 4:30 pm and would primarily be in our southern team although travel to other areas could be possible as deemed necessary.

Key Responsibilities

Manage health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services.

Assures that patient receives appropriate measures to control symptoms through collaboration with interdisciplinary team members.

Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.

Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations.

Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.

Education/Experience

Graduate from an accredited School of Nursing and accredited Nurse Practitioner Program with a minimum of one (1) year Nurse Practitioner experience

Current nursing licensure in State and CPR certification

Certification in a specialist area preferred; (e. g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)

Requirements

Candidates must be graduates of an accredited nursing and Nurse Practitioner program with at least one year of experience. A current nursing license and CPR certification are required, with specialist certifications like CHPN preferred.
