SUMMARY: A behavioral health case manager assesses, plans, implements, monitors care for individuals with mental health or substance use conditions. They coordinate services, advocate for clients, address barriers to care, and work to improve their overall well-being and independence.
Key Responsibilities
- Provides on
- going supportive and/or case management functions assisting other BH clinicians to equip their clients with needed internal and external resources and skill building.
- Partner with designated external and internal interdisciplinary providers, programs, entities to address the needs, gaps in care, and recovery goals of complex and high risk community members.
- Identify and remove barriers to procurement, delivery, participation in and success of services
Requirements
- Regularly communicate/consult with Client’s counselors, Clients’ treatment team, and/or Manager and Director regarding client’s behavior, progress and treatment goals.
- Participate in care team or child/family team meetings.
- Attend staff trainings and in
- Willing to learn more about the American Indian culture and attend at least one Cultural Presentation/Training per year.

- Hourly rate: $37.5/hr.
