We are hiring a Medical Social Worker (MSW) for a Home Health position providing skilled nursing care to patients in their homes in the Ingham and Eaton Counties area.
What You’ll Do
Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
Conduct comprehensive psychosocial assessments, identify patient and family needs, and develop individualized care plans that address emotional, social, and environmental factors
Educate and support patients and families by connecting them with community resources, counseling services, and care planning guidance to ensure they feel informed, supported, and empowered
Apply critical thinking and problem-solving skills to navigate complex situations, advocate for patient needs, and maintain accurate, timely, and compliant EMR documentation

- MSW Qualifications
- Master’s in Social Work from graduate school accredited by the Council of Social Work
- One (1) to two (2) years post graduate social work experience in a health care setting
- Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
