Key Responsibilities

Deliver compassionate, patient-centered care in a hospice setting while collaborating with an interdisciplinary team. Conduct comprehensive psychosocial and bereavement assessments to develop care plans that support emotional and end-of-life goals.

Requirements

Requires a Master’s in Social Work from an accredited graduate school and one to two years of post-graduate experience in a healthcare setting. A valid driver’s license is mandatory due to the field-based nature of the role.
