Join Guthrie as a Palliative Care Social Worker – Full-Time Days in Binghamton, New York. Full-Time Day Palliative Care Social Worker M-F 8-4:30. This role will include inpatient and outpatient work, participating in multidisciplinary rounds, patient and family meetings and counseling, assisting in coordinating transition to hospice services.
This position offers weekends off. Work with Guthrie in Binghamton, New York.
The social worker provides psychosocial evaluations, counseling, and support for patients and families in inpatient and outpatient palliative care settings. They also facilitate discharge planning and coordinate with the interdisciplinary team to ensure continuity of care.
Candidates must hold a Master's degree in Social Work and a valid New York State Clinical Social Work license. A minimum of two years of postgraduate experience in a healthcare setting is strongly preferred.
